ESRD Trans Clinical Social Wrk
Responsibilities Monitors clinical progression, reassess and adjusts plan of care accordingly. Utilizes the nursing process to assess, plan, implement and evaluate the discharge needs for patients in assigned caseload. Performs utilization management activities using approved guidelines. Provides follow up with appropriate resource for utilization management issues/non-compliance. Participates in quality improvement (QI) activities. Advances knowledge and skills in case management. Collaborates effectively with departmental staff to achieve goals and objectives.
Master's degree in Social Work with state Social Worker's license required. One to three years of social work in an ESRD setting required. Those hired before 1976 must have two years of social work experience, one year of which was in an ESRD setting, and must have a consultative relationship with a master's prepared social worker.
Other Information Regular full time (1.00 FTE) and regular part time (.50 FTE) position available.
