Physical Therapist UHC
Responsibilities Coordinates and manages Physical Therapy Services to patients in their homes. Provides individualized skilled care as indicated. Performs on-call duties as assigned. Assesses patients in their homes to determine level of physical function. Determines the nature of the physical dysfunction, the severity of the dysfunction, the prognosis for improvement, and recommendations for treatment. Establishes and implements a physical therapy treatment plan in collaboration with the patient's primary physician and the multidisciplinary team.
BA, MA or DPT degree in Physical Therapy and NC PT license required.
